Vial label units: what mg, mcg, and IU mean
The label states a total, and sometimes a unit that looks like it should convert the way mg and mcg do. IU doesn't — and knowing why keeps you from inventing a number that isn't on the label.

Two units can look like the same kind of number on a label and mean two completely different things. Knowing which is which happens before any math does.
mg and mcg both measure mass, and converting between them is division by 1,000. IU measures something else entirely, biological activity, and it doesn’t reduce to milligrams without a number that’s specific to the product in front of you.
The number on the vial is a total, not a dose
A vial printed “10 mg” holds 10 mg, full stop — everything in that container before you add any diluent. It isn’t a single injection amount, and it isn’t the number you draw up. That distinction is the most common misread on a label, and it’s the one that makes every later step wrong if you get it backward.
The total matters because it’s the input to a different calculation. Divide it by the diluent volume you add, and you get concentration — the number a single dose is actually drawn from. That’s the whole subject of reconstitution math: one vial total, one diluent volume, one concentration, computed once and used for every draw after.
mg and mcg are the same measurement, only the decimal moves
Milligrams and micrograms measure the same thing, mass, just at two different scales. A microgram is a thousandth of a milligram, so converting between them is division or multiplication by 1,000. Nothing more happens in this step.
Small as that step looks, it’s where most draw errors start. Skip it, or do it twice, and every number after it (concentration, draw volume, syringe units) is off by a factor of a thousand while still looking plausible on the page. Once the dose is in milligrams, turning it into syringe units is the next and only remaining step.
IU measures activity, not mass
IU stands for International Unit, and it isn’t a weight at all. It’s a measure of biological activity: how much effect a substance produces, defined separately for each compound against a reference standard for that compound’s action. A milligram is a fixed amount of matter anywhere in the world. An IU is fixed only relative to the substance it’s measuring.

That’s why 100 IU of one substance and 100 IU of another can correspond to entirely different milligram amounts — the unit was built to describe effect, not mass, and effect-per-milligram varies by compound.
Why there’s no formula from IU to mg
There’s no universal multiplier that turns an IU count into milligrams, and no calculator — including Amidra’s web tool — can supply one from the number alone. A few reasons that’s true, not a design gap:
- Different substances, different definitions. An IU of one compound is calibrated against a reference standard specific to that compound. There is no fixed IU-to-gram ratio that carries over to a different substance.
- It’s a potency figure, not a weight figure. Two preparations of the same substance can differ slightly in activity per milligram, which is exactly what IU is built to capture. It’s also exactly what a generic formula can’t.
- The conversion factor is product-specific, if it exists at all. Where one is published, it comes from that product’s own manufacturer documentation or reference material — never from a general rule you can memorize.

Reading the rest of the label without guessing
- Confirm the total amount and its unit — mg, mcg, or IU — and convert to mg first if it's stated in mcg.
- If the unit is IU, stop before assuming a milligram figure. Check the product's own documentation for a stated conversion, or ask the pharmacist or manufacturer directly. Don't estimate.
- Check the diluent or concentration statement, if the vial's already been reconstituted — it should match the number you calculated, not just a figure that looks close.
- Note the lot and expiration fields last. They don't feed the math, but they're part of confirming you're reading the label on the right vial.
Bottom line: read the unit before you read the number
A total isn’t a dose, a milligram isn’t a microgram until you divide by 1,000, and an IU isn’t a milligram under any circumstances a generic formula can reach. Three different mistakes, one shared cause — treating the label’s unit as a formality instead of the first thing to check. Read it first, and the arithmetic that follows is the easy part.

Not with a general formula.
