Converting a dose into units on a U-100 syringe

The last step of a draw is turning millilitres into the units printed on the barrel. Here's the conversion — and the two places it goes wrong.

Millilitres and units are the same volume. One is measured on a ruler; the other is that ruler cut into a hundred pieces.

A unit is just a hundredth of a millilitre

To put a dose onto a U-100 syringe, work out the draw in millilitres and multiply by 100. That’s the whole conversion. A U-100 syringe is graduated at 100 units per millilitre, so one unit is 0.01 mL — a hundredth of a millilitre, no more.

So 0.25 mL is 25 units. 0.5 mL is 50 units. It’s one volume, read on a finer scale.

The conversion, one line at a time

Getting to that millilitre figure takes two earlier steps. Convert the dose to milligrams, divide by the concentration you wrote on the vial, then multiply by 100. The example below uses a 10 mg/mL vial and a 250 mcg target — the same numbers you can type into the separate Amidra web calculator to check.

  1. Convert the dose to milligrams — micrograms ÷ 1,000. Do this before anything else; it's where most errors start.
  2. Divide by the concentration — dose in mg ÷ concentration in mg/mL gives the draw in millilitres.
  3. Multiply by 100 — millilitres × 100 is units on a U-100 barrel. Read the mark before you draw.

The middle step needs a concentration, which comes from reconstitution. If you haven’t fixed that number yet, start with the three reconstitution formulas and come back — this step picks up exactly where that one ends.

Read the right ruler

The arithmetic is the same on every U-100 syringe. What changes is how far 2.5 units sits along the barrel, because a 0.3 mL syringe and a 1.0 mL syringe pack their markings differently.

A unit is 0.01 mL on all of them — that never changes. The 3 mL row isn’t an insulin syringe, so its unit column is just the same 100-units-per-mL math extended; on that one, read the millilitre markings directly. And note that 2.5 units is most of the way up a 0.3 mL barrel but a short stub on a 1.0 mL one. Draw on the barrel you actually have, and read the number, not the visual level.

Where the two mistakes hide

Nearly every unit error is one of two slips. Both are easy to catch once you know to look.

  • The unit mix-up. Skipping the mcg-to-mg step, or doing it twice, throws the answer off by a factor of 1,000. Check which unit your vial label states the dose in first; if a draw comes out as 2,500 units or 0.0025, you converted units wrong — not the volume.
  • The wrong barrel. Reading 2.5 units on a 1.0 mL syringe as if the scale were the 0.3 mL one puts the plunger in the wrong place. The math was right; the ruler was wrong.

A unit error survives every check but the one that counts: does the mark on the barrel match the volume you calculated?

A worked check you can reproduce

Say you’ve been prescribed 5 mg from a compounded vial your pharmacy filled at 10 mg/mL. Same three steps, your own numbers:

  1. To milligrams — 5 mg is already in milligrams, so nothing to convert.
  2. Divide by concentration — 5 mg ÷ 10 mg/mL = 0.5 mL.
  3. Multiply by 100 — 0.5 mL × 100 = 50 units.

Fifty units is half of a 1.0 mL barrel, or the full length of a 0.5 mL one. Whichever you’re holding, the plunger stops at the volume the label and the math agree on — not at a number you remembered. Bottom line: same volume, two rulers

Units aren’t a different measurement from millilitres — they’re millilitres read on a hundred-part scale. Get the dose into milligrams, divide by concentration, multiply by 100, and confirm the mark on the barrel in your hand. The one habit worth keeping: never trust a unit figure you didn’t watch come out of those three steps.

Common questions

How many units is 0.5 mL on a U-100 syringe?

50 units. A U-100 syringe is graduated at 100 units per millilitre, so any volume in millilitres becomes units when you multiply by 100 — 0.5 mL × 100 = 50 units.
