What is Thymosin Alpha-1? A plain-English explainer

Thymosin alpha-1 is a naturally occurring peptide — a chain of 28 amino acids — first isolated from the thymus gland. A synthetic version made for pharmaceutical use is marketed abroad under the brand name Zadaxin (thymalfasin); it is not approved by the FDA and carries no marketing authorisation in the United States.

Thymosin Alpha-1 in plain English

Thymosin alpha-1 is 28 amino acids long, which puts it in peptide territory, alongside a whole family of related thymosin peptides first identified in extracts of the thymus gland.

The thymus is a gland, situated behind the breastbone, that is most active early in life and plays a role in the development of certain immune cells. Thymosin alpha-1 was one of the peptides isolated from thymus tissue and later characterized and reproduced synthetically. That synthetic version, thymalfasin, is manufactured as a pharmaceutical product and sold under the brand name Zadaxin.

Zadaxin holds marketing authorisation in more than 30 countries — including China and Italy — where its approved indication is primarily chronic hepatitis B. It does not hold FDA approval and is not an authorised medicine in the United States. Separately from the approved pharmaceutical product, thymosin alpha-1 is also sold in the U.S. as a research peptide, arriving as a small glass vial of white powder — the peptide with the water removed (freeze-dried, or “lyophilised”) so it stays stable in storage. On its own, powder can’t be measured into a syringe; that’s why reconstitution exists.

The science, a little deeper

A few specifics that the plain-English version glosses over:

  • It’s a defined sequence, not a mixture. Thymosin alpha-1 is a single, known 28-amino-acid sequence. That’s what makes it a peptide in the chemical sense rather than a crude tissue extract.
  • It’s naturally occurring, and also manufactured synthetically. The molecule was first identified in thymus tissue; the version used in the approved Zadaxin product and in research supply is made synthetically to match that sequence exactly.
  • Regulatory status differs by country. Thymosin alpha-1, as Zadaxin, is an approved prescription medicine in more than 30 countries for chronic hepatitis B. In the United States it has no FDA approval and no authorised indication; it is sold and studied there as a research peptide, and much of the published research outside its approved use is preclinical (cell and animal studies).
  • It’s supplied lyophilised. Removing the water gives the powder a long shelf life; adding a measured liquid back (reconstitution) is what turns it into an injectable solution with a known strength.

How reconstitution works for Thymosin Alpha-1

Reconstitution is just dissolving the powder in a known volume of liquid so you have a concentration — milligrams of peptide per millilitre of solution. Once the concentration is fixed, the volume you draw for any target dose is simple arithmetic:

  1. Concentration = vial amount (mg) ÷ diluent volume (mL)
  2. Draw volume = target dose ÷ concentration
  3. Syringe units = draw volume (mL) × the units-per-mL of your syringe

The numbers all come from your own vial label and your prescription — the peptide amount printed on the vial, the volume of bacteriostatic water you add, and the dose you were told to use.
